Sudden High Blood Pressure With No Symptoms? Here’s Your Calm and Safe At-Home Plan

A sudden spike in blood pressure that reaches very high levels but without symptoms is called hypertensive urgency. This can be scary, especially for older adults and their caregivers, but the good news is there are safe and effective ways to respond at home.

What Is Hypertensive Urgency?


When blood pressure shoots over 180/120 mm Hg without clear signs of organ damage like chest pain or vision changes, it is called hypertensive urgency. It is different from hypertensive emergency, which requires immediate hospital care.


Although hypertensive urgency is not an immediate life-threatening emergency, it still needs prompt attention to prevent complications.



Your At-Home Plan


1) Sit, rest, and recheck correctly. 

After the initial reading. Wait for 5 minutes then recheck. Follow the American Heart Association’s instructions for proper home measurement to ensure a proper reading.



2) Review possible culprits.


Think about the past few hours. Recent medications, nicotine, caffeine or alcohol, extra stress, can all raise blood pressure. Write down what you took or did so you can share it with your clinician.


3) Call your health professional if it remains high.

If your pressure stays around 180/120 or higher after a recheck and you have no symptoms, contact your clinician the same day for guidance.


Do Calming Techniques While Waiting


These do not replace medical advice. They help the body relax while you confirm readings and contact your clinician.


Practice slow breathing for 2 to 5 minutes.


Sit comfortably, breathe in through your nose for 4 counts, pause, breathe out for 6 to 8 counts. Slow, diaphragmatic breathing can lower heart rate and help stabilize blood pressure in the moment.



Rest your body.


If you feel tense, lie down or recline in a quiet spot and relax your shoulders and jaw. Rest while lying down can improve help your blood flow and reduce blood pressure. It also reduces the stress surge that can push numbers up. Sip water if you might be dehydrated.


Not drinking enough can trigger hormone and blood vessel changes that push numbers up. Take small sips of water. Avoid sports drinks or sugary drinks when rehydrating because those have sodium and sugar that can further increase your blood pressure.

What Not To Do


If prescribed blood pressure medication, take it exactly as directed. Do not double doses if you miss one.


Do not ignore a very high reading just because there are no symptoms. Confirm the number and call your clinician the same day.


When To Call 911


Call 911 right away if your blood pressure is 180/120 mm Hg or higher and you have any of these symptoms: chest pain, shortness of breath, severe headache, vision changes, weakness or numbness on one side, trouble speaking, confusion, back pain, or fainting. These can be signs of a hypertensive emergency. Do not wait.
