Through our partnership with Momentra, we are bringing innovative, technology-assisted support services.
Their Virtual Care Navigation program allows older adults to access tools such as video calls and messaging, an interactive health map, a secure file vault for personalized health information, and activities designed to support overall well-being.
Every participant is supported by a dedicated Care Ambassador who provides ongoing virtual engagement through weekly video meetings, regular phone calls, and daily SMS check-ins while helping coordinate essential services such as transportation, grocery delivery, medication support, appointments, and meaningful community involvement.

Connect with a medical social worker for personalized support and resource referrals, including VA benefits, IHSS, meals, transportation, and more. Sliding scale fees apply for one-time or ongoing in-person visits.

Community Health Workers
CHWs help individuals access vital resources like housing, food, and healthcare, while promoting healthier lifestyles through culturally relevant education, outreach, and advocacy.
For private-pay clients, the service is offered as case management at $60/hour, billed in 15-minute increments, so a 15-minute session with a social worker can be just $15.
