They do personal assessments through in-person visits or online calls to address each person's unique circumstances.
They work alongside your primary care providers to help you reach your health goals, navigate complex systems, and connect with the resources that make lasting wellbeing possible.

FREE Community Health Worker Assessment
Many people with Medi-Cal providers Kaiser Permanente, Inland Empire Health Plan (IEHP), and Community Health Plan of Imperial Valley (CHPIV) are surprised to learn that they may qualify for free or low-cost support services that can make daily life safer and more manageable.
Unfortunately, these benefits often go unused because they are not well explained, not widely advertised, or simply hard to navigate on your own.
If you or someone you care for has is a member of any of those three organizations, we encourage you to reach out to us for a free assessment.
During the call, our Community Health Workers will ask a few simple questions to better understand your situation and help identify services and benefits you may already qualify for, but may not even realize are available.
There is no cost for this assessment, and no obligation to move forward.
Depending on eligibility and approval, support may include:

CHW Pricing
How Much Do CHWs Cost?
For eligible individuals, they can get Community Health Worker services for FREE while private pay clients pay
Private Pay
For private pay clients, the service is offered as case management at $60/hour, billed in 15-minute increments so a 15-minute session with a social worker can be just $15.
Medi-Cal Covered Services
If you're a member of Medi-Cal providers Kaiser Permanente, Inland Empire Health Plan (IEHP), and Community Health Plan of Imperial Valley (CHPIV), you can get Community Health Worker services at no cost.
