Respite
Provides relief for primary caregivers. Respite allows caregivers to get a break while we provide the needed care (up to 336 hours per calendar year).
Free Community Health Worker Assessment
Many members of Kaiser Permanente, Inland Empire Health Plan, and Community Health Plan of Imperial Valley may qualify for free or low-cost support services that make daily life safer and easier.

Depending on eligibility, this can include:

These benefits often go unused because they are hard to find or navigate.
Our Community Health Workers will ask a few simple questions and help identify services you may already qualify for at no cost and no obligation.
