Our Community Health Workers can help you identify services and benefits you may already qualify for but may not realize are available including:

Other supportive services designed to help people remain safe at home
Respite
Provides relief for primary caregivers. Respite allows caregivers to get a break while we provide the needed care (up to 336 hours per calendar year).

Respiro para Cuidadores (Respite)

Free Community Health Worker Assessment
Muchas personas con proveedores de Medi-Cal como Kaiser Permanente, Inland Empire Health Plan (IEHP) y Community Health Plan of Imperial Valley (CHPIV) se sorprenden al saber que pueden calificar para servicios de apoyo gratuitos o de bajo costo que pueden hacer la vida diaria más segura y manejable.
