San Diego County Area Agency on Aging

Provides programs and services to help older adults and persons living with disabilities to stay safe, healthy, engaged, and as independent as possible. Contact their call center to know all about the services in the county.

In-Home Supportive Services

Provides homemaker and personal care assistance to eligible individuals who are receiving Supplemental Security Income or who have a low income and need help in the home to remain independent. Assistance can include bathing, grooming, shopping, laundry, and meal preparation.

Multipurpose Senior Services Program (MSSP)

A care management service for seniors age 65 and older who are eligible for Medi-Cal and at risk of institutionalization.

Linkages Program

Works with older adults to find and coordinate services in the home. Linkages staff develop a care plan with the older adult and any significant others to help the client remain safe and as independent as possible. One goal is to prevent hospitalization or long-term, out-of-home placement.

Senior Options, Advocacy and Referrals (SOAR)

A care management program that serves frail and disabled adults age 60 and older, who are at risk of nursing home placement and ineligible for other care management and in-home-care service programs.

Family Caregiver Support Program

Provides support and services for caregivers. These services include care management, respite care, legal services, counseling, minor home modification, and more.

Meals & Dining Centers

The County's Aging & Independence Services (AIS) department (with funding from the Older Americans Act) contracts with several nutrition centers to provide hot, nutritious lunches during the week, for adults age 60 and older.

Home-Delivered Meals

Adults 60 and older who are homebound due to illness or disability can ask to have meals delivered to them if they are registered with a County of San Diego nutrition provider that offers this service.

CalFresh Healthy Living

Program aims to empower Cal-Fresh eligible communities lead healthy and active lifestyles by providing nutrition education, and supporting policy, system, and environmental changes that can improve access to healthy food and physical activity opportunities.

Fall Prevention Task Force

A community-based coalition composed of healthcare professionals, senior service providers, and community advocates. It's mission is to reduce falls and their devastating consequences in San Diego County.

Healthy Living Programs

The county has multiple classes older adults can attend to to manage chronic conditions, improve physical functioning, increase socialization, and prevent falls. There are a variety of ways these programs are delivered. For all the details, please visit their website.

Adult Protective Services

Serves adults 60 and older and dependent adults 18 and older, who are harmed, or threatened with harm, to ensure their right to safety and dignity.

Long-Term Care Ombudsman

advocates for residents in long-term care facilities, such as nursing homes, as well as investigates abuse in other licensed facilities.

Project C.A.R.E.

Identifies and supports older adults and/or individuals with disabilities to prevent social isolation, loneliness, neglect, and abuse.

Provides a crisis assessment and assistance for persons age 60 and older with mental health concerns.

Health Insurance Counseling & Advocacy Program (HICAP)

Offers counseling regarding Medicare, Medicare supplemental insurance (Medigap), HMO options, long-term care insurance, and billing problems.

Elder Law & Advocacy

Provides adults age 60 and older with an affordable way to take care of simple legal matters. Referrals are available for more complex legal assistance. Family caregivers of persons 60 and older may also receive free assistance.

Senior Transportation Program

Provides safe and reliable transportation for older adults living in specific areas at no cost to the rider. Each eligible rider will receive up to 100 miles worth of rides each month.

Employment Training

provides funding for some employment training for low-income older adults.

Senior Employment Training (SCSEP)

A community service and work-based job training program for low-income, unemployed older adults. SCSEP participants gain work experience in a variety of community service activities at non-profit and government agencies.

Comprehensive & Professional Care

We provide comprehensive support, including personal care, mobility assistance, housekeeping, communication, and transportation. 

All of our caregivers undergo background checks and receive training and certification through the Caregiver Training University.

Our private pay in-home care services are $28 per hour, with a two-hour minimum per visit, no long-term contracts
